If you’re wondering when Mounjaro® should start making a difference, you’re not being impatient. Losing weight may have already taken up too much of your time, effort and headspace in the past. It makes sense to want to know whether this treatment is actually doing something.
Mounjaro® starts working in your body after your first injection. Within the first few days, it can begin reducing your appetite, helping you feel full sooner and quieting thoughts about food. Weight loss takes longer and usually becomes easier to see after 8 to 12 weeks. If you feel little on 2.5mg or see little change in your first month, it does not mean Mounjaro® will not work for you.
What to Expect in Your First Month on Mounjaro®
Your first month is mainly about starting safely and giving your body time to adjust. Mounjaro® is already active during this period, even if the effect feels subtle. The level of tirzepatide, its active ingredient, is usually highest 8 to 72 hours after an injection. It then builds with each weekly dose and reaches a steady level after about 4 weeks*.
That is why 1 injection cannot tell you how well treatment will work for you.
How much weight can you lose in the first month?
There is no single first month result you should reach. Some people lose weight in their first 4 weeks, while others see little or no change. Early results depend on your starting weight, appetite response, side effects, menstrual cycle, other health conditions and how much normal water fluctuation is hiding the trend.
Month 1 is not a pass or fail test. It is the beginning of a gradual treatment process. Appetite changes, smaller portions and a shift in how your clothes fit can all be useful signs before the scales show a clear pattern.

What your first 4 weeks might look like: A timeline
Week 1: After your first injection
Mounjaro® starts working in your body after your first dose. You might notice that you feel hungry less often or you feel satisfied with less food or thoughts about food feel less demanding, but it’s also normal to not experience any changes at all at this stage.
Nausea, diarrhoea, constipation or stomach discomfort can also begin early, although not everyone has side effects. Feeling sick is not a sign that Mounjaro® is working better. It shows how your body is tolerating the medication*.
Weeks 2 to 4: As Mounjaro® builds in your body
With each weekly dose, the amount of Mounjaro® in your body builds. Appetite changes may become clearer. You might leave food on your plate without planning to, snack less often or stay full for longer after a meal.
You remain on 2.5mg for these first 4 weeks. This is the starting dose, designed to help your body adjust before moving to 5mg. Some people respond strongly to 2.5mg. Others feel very little. Don't take an extra dose or increase early because you cannot feel it working*.
Accordion: When will I feel Mounjaro® working?
Some people feel appetite suppression within the first few days. For others, it takes several weekly doses or a dose increase before the difference becomes obvious. The change can be quieter than expected. Rather than feeling that you have no appetite at all, you may simply find it easier to stop eating when you have had enough.
Appetite suppression can also feel stronger on some days than others. Tirzepatide has a half life of about 5 days, so you may notice more hunger as your next injection approaches. This does not necessarily mean the treatment has stopped working*.
*Eli Lilly and Company Limited. (2026). Mounjaro® KwikPen®: Summary of Product Characteristics (SmPC).
Use extra contraception when you start Mounjaro®
If you are overweight or living with obesity and take an oral contraceptive, use condoms for 4 weeks after starting Mounjaro® and for 4 weeks after every dose increase. You can also speak to a healthcare professional about changing to a method that is not taken by mouth. Vomiting and severe diarrhoea can reduce protection from the pill too. Follow the missed pill advice for your specific pill if either happens.
Mounjaro® Results Month by Month
Once you have completed your first 4 weeks, progress usually becomes easier to judge over months rather than individual days. This timeline is a guide, not a target. Your dose, starting weight, side effects, health and individual response all matter.
Mounjaro® Results Month 2
The licensed dosing schedule moves from 2.5mg to 5mg after 4 weeks. If your dose increases, appetite suppression may become clearer. You may feel full sooner, find portions easier to manage or notice less interest in snacking. Digestive side effects can also appear or return after a dose increase, although they often reduce over time.*
For many people, their weight progress becomes easier to see during this month. Others still need more time. A slower start does not mean you are resistant to Mounjaro® or that treatment has failed.
Mounjaro® Results Month 3
Your dose now depends on your response and what you can comfortably tolerate. Dose increases are made in 2.5mg steps, and you need to stay on each dose for at least 4 weeks. Depending on how well your dose works for you, you may stay on the same dose or discuss an increase with your clinician*.
By now, hunger and fullness may feel more predictable and your weight trend may be clearer. Weight loss will not be identical each week. A week with no movement can sit within an overall downward trend. Don’t only focus on the scales; changes to how your clothes fit or to your measurements are as important.
*Eli Lilly and Company Limited. (2026) Mounjaro® KwikPen®: Summary of Product Characteristics (SmPC).
Mounjaro® Results Months 4 to 6
By this stage, the changes may be easier to see. In a real world study, people who stayed on tirzepatide for at least 6 months lost 12.9% of their starting weight on average*, although individual results vary. You might also notice changes in your measurements, looser fitting clothes or more consistent appetite control. Progress is not linear.
Your current dose may continue to work well, or you and your clinician may decide that a gradual increase is appropriate. You do not need to increase your dose simply because a higher dose exists. If your current dose is supporting progress and side effects are manageable, staying on it may be the right choice.
You may also have periods when weight loss slows. That can happen as your body gets smaller and needs less energy. Look at several weeks together before calling it a plateau.

Beyond 6 months
Mounjaro® is designed as a longer term treatment when it remains suitable for you. Appetite effects may feel less dramatic because they have become familiar, not because the medication has necessarily stopped working.
In a 72 week clinical trial, average weight loss reached 22.5% among people taking 15mg Mounjaro® alongside diet and exercise. That is an average from a specific trial group, not a result everyone will reach. People taking lower doses also lost weight on average, and individual results varied.*
The official review point is not simply six months after your first injection. If you have lost less than 5% of your starting weight 6 months after reaching your highest tolerated dose, your clinician should review the benefits and risks with you and decide whether continuing is right for you.**

How Can I Tell Whether Mounjaro® Is Working?
The first signs often show up in everyday decisions before they show up as a dramatic number on the scales. You might notice:
You feel hungry less often.
You feel satisfied with a smaller portion.
You stay full for longer after eating.
Cravings or thoughts about food feel quieter.
You snack less without having to fight the urge.
Your waist measurement changes or your clothes fit more loosely.
Walking, exercising or everyday movement feels more comfortable.
These signs don't all have to happen at once. You also don't need nausea, diarrhoea or another side effect for Mounjaro® to work. Side effects tell you about tolerability, not how much weight you will lose.
Why Am I Not Losing Weight on Mounjaro® Yet?
It can be deeply frustrating to start treatment and see less change than you hoped for, especially if losing weight has already felt difficult for years. A slow start does not mean you have done something wrong.
Still in your first month?
The 2.5mg dose helps your body adjust. Some people respond strongly at this dose, while others don't notice much until they have taken several doses or moved to a higher dose with clinical guidance. Your first month cannot predict your final result.
Appetite has changed, but your weight has not?
Feeling less hungry is an early sign, but it can take time for that change to create a visible weight trend. Your waist measurement, portion sizes, cravings and how your clothes fit can help you see progress that the scales have not shown yet.
Taking Mounjaro® consistently but seeing no progress?
Missed doses, difficulty using the pen or a dose that is not giving you enough benefit can affect progress. A clinician can review what you are experiencing and whether a change is appropriate. Higher doses produced greater average weight loss in trials, but the right dose is not automatically the highest one. It should give you useful benefit with side effects you can manage.*
*Jastreboff AM, et al. (2022) Tirzepatide Once Weekly for the Treatment of Obesity.
Lost weight at first, but now nothing is changing?
Weight loss often slows after the earlier stages of treatment. One unchanged week is not a plateau. If your weight and measurements have stayed broadly the same for several weeks, your clinician can review your dose, appetite, side effects, eating pattern, movement and health history with you.
Does Mounjaro® Work Differently for Women?
Women are not one group with one predictable response to Mounjaro®. Your menstrual cycle, reproductive stage, contraception and other medicines or conditions such as PMOS may affect your experience of treatment or need to be considered alongside it.
However, these factors don't mean there's a separate timeline that your weight-loss progress has to follow. Everyone responds differently, and your Hers clinician can help you understand what to expect based on your individual circumstances.
Mounjaro® during perimenopause or menopause
Menopause does not appear to stop Mounjaro® from supporting weight loss. In a post hoc analysis of women from the SURMOUNT programme*, women experienced substantial weight loss across premenopause, perimenopause and postmenopause groups.⁺
The analysis looked at weight loss at the end of the treatment period, so it can't predict exactly how quickly an individual woman will respond.
If you take hormone replacement therapy (HRT), tell your clinician what type you use. There isn't enough evidence to say that HRT makes Mounjaro® work faster or slower.

⁺Tchang BG, et al. (2025) Body weight reduction in women treated with tirzepatide by reproductive stage: a post hoc analysis from the SURMOUNT program.
How quickly does Mounjaro® work if you have PMOS?
There is no good evidence that Mounjaro® takes longer to work if you have PMOS (previously known as PCOS). A 2026 UK study found similar average weight loss at 10 months in women with self reported PMOS and women without PMOS. Only a small number of women with PMOS still had data at that point, so the result should not be treated as a promise about individual progress*.
Mounjaro® is not licensed specifically to treat PMOS. It may still be prescribed for weight management if you meet the relevant criteria. Weight loss and improved metabolic health may also affect ovulation, so pregnancy can become possible even if your periods have previously been irregular.
*Clift AK, et al. (2026) Weight loss outcomes with tirzepatide in women with and without self reported polycystic ovary syndrome.
Can Mounjaro® change your periods?
Period changes are not listed as a common direct side effect of Mounjaro®. However, your cycle can change while you are losing weight. Weight change, shifts in body fat, changes in insulin resistance, PMOS (previously known as PCOS) and contraception can all play a part. Your period may arrive earlier or later, feel heavier or lighter, or include spotting. Track what happens over a few cycles. Speak to a clinician if bleeding is very heavy, changes continue, you have unusual pelvic pain or you are concerned. Take a pregnancy test if your period is late and pregnancy is possible.
Does Mounjaro® affect the contraceptive pill in the first weeks?
Yes, it may reduce how well an oral contraceptive works in women who are overweight or living with obesity. If you take the pill, use condoms for 4 weeks after starting Mounjaro® and for 4 weeks after every dose increase. Alternatively, speak to a healthcare professional about a method that is not taken by mouth*.
Vomiting and severe diarrhoea can also reduce protection from the pill. If you vomit within three hours of taking it, or have severe diarrhoea for more than 24 hours, follow the missed pill instructions for your specific pill. Ask a pharmacist, sexual health service or clinician if you are unsure**.
Mounjaro® should not be used during pregnancy. If you become pregnant while taking it, stop treatment and speak to a healthcare professional as soon as possible*.

What Can Support Steady Progress on Mounjaro®?
Mounjaro® is there to make weight loss more manageable by reducing hunger and helping you feel full sooner. You still need consistency, but you should not have to push your way through every meal. These habits can help you work with the medication:
Take your injection on the same day each week
Follow the dose plan agreed with your clinician.
Build regular meals around foods that provide protein and fibre, adjusting portions to your appetite.
Drink enough fluid, particularly if nausea, vomiting, diarrhoea or constipation is affecting you.
Choose movement you can repeat. Walking and strength based exercise can both support health while you lose weight.
Protect your sleep where you can. Poor sleep can make hunger and daily routines harder to manage.
Use your clinical support. Side effects, plateaus and dose questions are reasons to check in, not reasons to struggle alone.
The aim is not to create another punishing set of rules. It is to use the quieter appetite Mounjaro® can provide to build choices that feel possible to repeat.
When to Speak to Your Clinician
You don't have to wait for a fixed milestone before asking for help. Speak to your clinician if:
You have felt no change in appetite and seen no weight trend after several months and dose reviews.
Your weight and measurements have been stable for several weeks after previously changing.
You are unsure whether to stay on your dose or discuss an increase.
Side effects are persistent, difficult to manage or stopping you from eating and drinking enough.
Your periods have changed and you are concerned.
You need advice about contraception, pregnancy or another medication.
For weight management, the official review point is not simply six months after starting. If you have lost less than 5%* of your starting weight 6 months after reaching your highest tolerated dose, your clinician should review the benefits and risks with you and decide whether continuing treatment is right for you. *Eli Lilly and Company Limited. FAQs
Can You Lose Weight on 2.5mg of Mounjaro®?
Yes, some people lose weight on 2.5mg. However, it is the starting dose rather than a licensed maintenance dose. The licensed schedule moves to 5mg after 4 weeks. Follow the plan agreed with your clinician and don't increase your dose early*.
*Eli Lilly and Company Limited. Mounjaro® KwikPen®: Summary of Product Characteristics (SmPC).
What is The Average Weekly Weight Loss on Mounjaro®?
There is no reliable weekly average because weight loss is not linear and doses change during treatment. In the main 72 week trial, people taking 15mg lost 22.5% of their starting weight on average, with reduced calorie intake and regular exercise. But remember, this is a trial average, not a promise about individual results*.
*Jastreboff AM, et al. (2022) Tirzepatide Once Weekly for the Treatment of Obesity.
How Long Does Mounjaro® Appetite Suppression Last?
Mounjaro® is taken once a week and remains in the body across the week. Some people feel appetite suppression throughout the week. Others notice it is strongest in the days after their injection and becomes less noticeable before the next dose. Its half life is about 5 days, and the effect may feel steadier once the medication has built up over 4 weekly doses*.
*Eli Lilly and Company Limited. Mounjaro® KwikPen®: Summary of Product Characteristics (SmPC).
Why Does Mounjaro® Feel Weaker Before My Next Injection?
It can feel weaker because the amount of tirzepatide in your body gradually falls between injections. Don't take your next injection early to compensate. If this happens regularly and is affecting you, discuss it with your clinician at your next dose review*.
